This is a teaching example, not a national survey, weekly total, risk rating or
current coverage assessment. The source contains nine rows for seven distinct
USDOT numbers: three BMC-35 records and six BMC-91X records marked TERM/REPL.
Repeated USDOTs are retained because one carrier may have multiple filings.

Transformation: retain every row in source order; convert YYYYMMDD dates to
YYYY-MM-DD; preserve empty values; derive the carrier URL. Policy identifiers,
insurer names and coverage amounts are omitted from the readable export but
remain in the raw file. No coverage or regulatory conclusions are added.

USDOT 2082074 has both a BMC-35 row with cancellation effective September 21 and
a BMC-91X TERM/REPL row with policy effective September 21 and cancellation
effective September 25. Those are separate fields, not one common “effective
date.” These rows alone prove neither continuous coverage nor a lapse.

## Reproduction and corrections

From the VerifyCarrier repository: `python3 scripts/data/build-insurance-example.py`.
Use `--check` to verify the frozen source SHA-256 and generated files without writing.
The hash is in the manifest. Changed input fails rather than silently replacing
the evidence. This file is deliberately frozen; it does not update daily.

row. Version 1 is the initial release; no corrections recorded.
